How to Submit Your Photo

In order to create your Service Dog product(s), we first need a photo of your dog. You can submit a photo to us in one of three ways:

1. EMAIL THE PHOTO TO US

If you have a digital image of your service dog (scanned image or digital photo), you can email it, along with your name, to photos@nsarco.com after you’ve completed your order.

NOTE: Many cell phone cameras produce poor pictures, and we don’t recommend using them unless you have no other way to take a picture of your dog.

2. U.S. MAIL

Order online and mail your photo to us. We’ll scan the photo for you, crop and use the image for your service dog ID, and the return the photo with your order. There’s no extra charge for this service. Remember to mail your order name and order number along with your photo.

Our mailing address is:

National Service Animal Registry

3. Print & Order by Fax or Mail

Prefer not to send your information over the internet? No problem! Just click this link, print, and complete the form and mail or fax it to us along with the photo.

You can fax the form to us: (719) 531-7719, but you’ll still need to mail or email us the photo.

You can also just mail in the order form and photo together.

Once we receive your information, we’ll make your service dog product and return your original photo with your order!

Photo Tips

Instructions
GOOD PHOTOS: This is an example of a good photo – and you don’t need to worry about extra space around the dog’s head because we’ll crop it for you (as seen by the yellow crop window).
Instructions

BAD PHOTOS: We do what we can to make your service dog product look good, but we’re not magicians! If you send us a poor photo like this one, it will still look bad on your service dog product.

Instructions

SILLY PHOTOS: If you send us a picture like this one, we’ll use it, but people won’t take your service dog seriously (no matter how adorable he looks)!

Instructions

POORLY SHAPED PHOTOS: If your photo is wide with a low height, you won’t be maximizing the available space for your service dog’s photo. The yellow border in the sample photo indicates the space for your dog’s picture. If you send us a “landscape” picture like this one, all the area in yellow is wasted and your dog’s photo will be smaller.

Instructions

Speaking of small, if you send us a tiny picture, it will be blurry on the service dog product.
